Not every flawed onion is a lost cause. A small isolated brown or wet spot can often be cut away cleanly if the flesh around it is firm and smells normal. Trim well past the bad bit and use the rest. A sprouting onion is also still safe to eat, just milder and a touch softer than a fresh one.
But once decay spreads, it’s not worth the gamble. Get rid of an onion if you see any of these:

One habit that’s saved me plenty of onions: check the stash about once a week. One rotting onion can spread to its neighbors fast, and a quick look catches the soft one before it takes the rest with it.
Before you cut, run the whole check in a few seconds. Look it over for spots and mold, press it to make sure it’s firm, then sniff near the root end for anything sour. Firm and sharp-smelling means you’re good to go.
